I am by no means an expert on Tin eye but from my understanding it does an extensive web search to find other sites with a matching picture which simply allows you to draw more informed conclusions regarding the picture. As for why people used fake pictures if tin eye is so easy to use... well like all the phone scams... they hope they will find enough people who will fall for it.
